We have physician services for patients who do not have their own primary physician and does home visits for those unable to leave their home. The services provided are primary care physicians, podiatry, and wound care specialists.
Patients with high nutritional requirements and who score 6 or more on the OASIS nutritional assessments are referred and seen at home by registered dietitians. Nutritional reassessments and follow-up teachings are given to patients and family. Patients who benefit the most on these services are our diabetic, CHF, renal, and patients with enteral feedings.
Durable equipments are provided by the agency's contracted companies and delivered directly to the patient's house.
Blood and other specimen needed are collected and tested in our assigned laboratory facilities. Radiologic testing also available.
